Why didn't the "If approved" branch run?

Theo's flow has a Condition after the approval: Outcome is equal to 'approve' (lowercase). Yuki approves a high-value renewal. The Condition takes the If no branch, sending a "renewal declined" notification to Rashida by mistake.

What did Theo get wrong?
